Our Care Managers seek to ensure supportive relationships with each client so they develop confidence in their own ability to live independently.

Assessment & Development of a Care Plan to include physical, emotional, mental, social, spiritual, financial, medical, and legal concerns of the client and/or family.

 

Referral to legal, financial, and/or medical professionals, and community services.

 

Assistance in Place of Residence for meals, companions, transportation, chores, etc.

 

Coordination of Alternative Living Arrangements in a retirement community, long-term care facility, or long-distance moves.

 

Coordination of Community Referrals & Monitoring to assure quality of services to the client, including assistance with medical appointments and communication with physicans and other medical services.

 

Crisis Response on a 24-hour, seven-day basis.

 

Money Management including assistance with bill paying, insurance filing, Social Security, Medicare, Medicaid, Veteran’s benefits, record keeping for client and family to insure continuity of care.

 

Communication to facilitate planning.

 

Consultation & Planning for Advance Directives: Living Will, Health Care Surrogate, Power of Attorney, Guardianship, Burial Planning.
